An aptitude examination or appraisal serves the purpose to eliminate existing doubts regarding personal aptitude for owning a firearm. In case such doubts exist or certain other conditions are present, the respective governmental department is required to withdraw an exitsting gun license or demand an aptitude certificate from the applicant.
The conditions that raise doubts about the personal aptitude are listed in Section 6 of the German Weapons Act.

In such a case, the respective department will ask you to submit a certificate confirming your physical and psychlogical aptitude, issued by officially appointed medical personnel, a medical or psychological specialist at your own expense.
In addition, anyone under the age of 25 will also be required to submit a certificate, confirming their aptitude at their own expense, for the initial issue of a permit to purchase and possess a firearm.
Doubts regarding personal suitability can be dispelled with the help of an examination/expert opinion under firearms law. If certain conditions are met or if there are doubts about personal suitability, the competent weapons authority is obliged to revoke the permit or demand a certificate of suitability.
Section 6 of the Weapons Act lists certain conditions that give rise to doubts about personal suitability.
According to Section 6 of the Weapons Act, you do not have the required personal suitability if there are indications that you
1. are legally incompetent,
2. are dependent on alcohol or other intoxicating substances, mentally ill or mentally retarded or
3. due to personal circumstances, you are unable to handle weapons or ammunition carefully or properly or are unable to store these items carefully or that there is a specific risk of endangering others or yourself...
In this case, the competent authority will ask you to submit an official or specialist medical or psychological certificate of mental or physical fitness at your own expense.
In addition, persons who have not yet reached the age of 25 must also submit an official medical or specialist medical or specialist psychological certificate of mental aptitude at their own expense for the first issue of a permit to purchase and possess a firearm.

If the issuing/controlling department has reasonable doubt that you are unfit to purchase and own a firearm , for example due to alcohol or drug addiction (see § 6 WaffG).
As stated in § 6 of the German Gun Law (WaffG) all expensese associated with the aptiude assessement have to be covered by the applicant.
